LETTERS Prolonged hemiplegic migraine associated with unilateral hyperperfusion on perfusion weighted magnetic resonance imaging

چکیده
Hemiplegic migraine (HM) is an unusual subset of migraine with aura, in which headache is associated with unilateral motor deficits, thought to be attributable to an underlying calcium channelopathy. In some cases the neurological dysfunction may outlast the headache and persist for many days. In the initial stages, hemiplegic migraine may mimic cerebral infarction. Within the first few hours after stroke, both computed tomography and magnetic resonance imaging (MRI) are often normal. However, more information can be gained using diffusion (DWI) and perfusion weighted imaging (PWI), which are much more sensitive to acute events in cerebral ischaemia. A recently reported postprocessing technique (factor analysis of dynamic studies FADS) can be applied to PWI to generate images representing arterial (“early”) and venous (“late”) contributions to signal intensity. This report outlines the findings arising from the application of multimodal MRI techniques to a patient with prolonged hemiplegic migraine.

A case of hemiplegic migraine in childhood: transient unilateral hyperperfusion revealed by perfusion MR imaging and MR angiography.
We report on an 8-year-old girl with a typical attack of hemiplegic migraine, in whom MR angiography and perfusion MR imaging revealed unilateral dilation of branches of both the middle and posterior cerebral arteries and hyperperfusion of the ipsilateral hemisphere, respectively. The findings resolved spontaneously after the attack. These imaging techniques should be indicated for patients wit...
متن کاملAcute striatal necrosis in hemiplegic migraine with de novo CACNA1A mutation.
We report the case of a 9-year-old girl with early-onset developmental delay, chronic ataxia and prolonged hemiplegic migraine episodes bringing about progressive deterioration. Two days into one episode, diffusion-weighted magnetic resonance imaging disclosed unilateral striatal abnormal signal consistent with cytotoxic edema, which evolved into atrophy on follow-up scans. Mutational screen of...
متن کاملArterial Spin Labeling Perfusion Magnetic Resonance Imaging Performed in Acute Perinatal Stroke Reveals Hyperperfusion Associated With Ischemic Injury.
BACKGROUND AND PURPOSE Perfusion-weighted imaging in adults with acute stroke often reveals hypoperfusion in the ischemic core and in a surrounding area of nondiffusion-restricted penumbral tissue. Perinatal stroke is common, but the perfusion pattern is rarely documented. We aimed to describe the perfusion pattern in newborns with perinatal stroke. METHODS Neonates with clinical features of ...
متن کاملProlonged hemiplegic migraine.
Hemiplegic migraine is a rare form of migraine variant. It is characterized by recurrent attacks of headache associated with temporary neurological deficit, usually unilateral hemiparesis or hemiplegia. It can be difficult to distinguish from migrainous stroke clinically, and a full neurological work-up and careful review of medical history and symptoms are necessary for the diagnosis. Two form...
متن کاملProlonged symptoms in sporadic hemiplegic migraine: aura or migrainous infarction?
PURPOSE Sporadic hemiplegic migraine is a rare form of migraine associated with motor weakness during the aura phase. While the aura usually lasts less than 1 hour, patients with sporadic hemiplegic migraine frequently have prolonged weakness. CASE REPORT A 60-year-old male had sporadic hemiplegic migraine after a head injury at the age of 14. He presented to our emergency department with a t...
